ChenMed, a physician-led and mission-driven primary care organization, is seeking a Nurse Practitioner to join their team. The role involves providing care for patients with acute and chronic illnesses, primarily older adults, with a patient panel of up to 450 patients. The Nurse Practitioner (PCP) is accountable for patient outcomes, clinical care, and cost-effectiveness, acting as a health coach to help patients set and achieve health goals. Responsibilities include assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation. The PCP leads a care team and collaborates with hospitalists, transitional care teams, and other healthcare professionals. The position requires adherence to departmental goals, regulatory compliance, and quality patient care standards. The PCP will also participate in risk and quality management programs, clinical meetings, and center management activities, including recruiting patients and providers. The role demands availability and accessibility to patients, evidence-based medicine practice, leadership development, quality improvement, and influence on patients and teams. Proficiency in Microsoft Office and willingness to travel locally and nationally for training and recruiting are required. Education and experience criteria include an ARNP or similar advanced nursing degree, current Nurse Practitioner certification, preferred board certification by AANP or ANCC, DEA number if required by state, at least 1 year of clinical experience preferred, and Basic Life Support certification within 90 days of employment.
