Summary:

Join our dynamic team at CHRISTUS SETX Health System , where you will play a vital role in delivering comprehensive care to our patients. As a Nurse Practitioner (NP) , you will collaborate closely with our Medical Director to ensure that every patient receives personalized care tailored to their needs. Your responsibilities will include:

• Coordinating patient management and developing individualized care plans.
• Conducting patient education to empower individuals in their health journeys.
• Ensuring accurate and timely documentation in line with regulatory standards.
• Participating in community events and marketing opportunities to promote our health system.

In this role, you will take medical histories, perform physical exams, order and interpret laboratory studies, and provide treatment for chronic diseases. You will work hand-in-hand with our dedicated RN and Medical Director to:

• Make follow-up calls to ensure continuity of care.
• Identify additional visits or interventions that may be necessary.
• Manage and adjust medications while educating patients on their treatment plans.

Requirements:

• Current Licensure through the Texas Board of Nursing OR Licensed Physician Assistant by the Texas State Medical Board (or the ability to obtain prior to employment).
• Valid DEA license (or the ability to obtain one prior to employment).
• BLS certification from the American Heart Association.
